Who can make referrals?

Any Health Care Professional, both community and Hospital based, can make a referral to Michael Sobell Hospice, following assessment of the patient. We do not accept self referrals directly from patients.

Referral Criteria

We do not operate a first come first served system but prioritise referrals on an appropriate type and severity of patient’s needs.

Referrals are accepted for patients who have a complex specialist palliative care need, either physical, psychological, spiritual or social.

Referral Coordinator days of work: Mon-Fri 8.30-5.00pm ( no weekends or BH )

For out of hours referrals/enquiries please call 020 3826 2377

Meetings are held regularly to discuss and prioritise admissions. Once accepted for admission, the referring healthcare professional will be contacted so they can finalise arrangements for transfer of patients to Michael Sobell Hospice.
