Who can make referrals?

Any Health Care Professional, both community and Hospital based, can make a referral to Michael Sobell House, following assessment of the patient. We do not accept self referrals directly from patients.

Referral Criteria

We do not operate a first come first served system but prioritise referrals on an appropriate type and severity of patient’s needs.

Referrals are accepted for patients who have a complex specialist palliative care need, either physical, psychological, spiritual or social.

Referral process

There is a dedicated referral line, tel: 07900 228036 which is available between 9am and 5pm from Monday to Friday (excluding Bank Holidays). Please call to discuss appropriateness of admission, to ascertain bed availability or to receive advice re patient management.

Out of hours admissions are considered on an individual basis, please call this number to discuss patient needs 020 3826 2377.

Meetings are held regularly to discuss and prioritise admissions. Once accepted for admission, the referring healthcare professional will be contacted so they can finalise arrangements for transfer of patients to Michael Sobell Hospice.
